Prior to varsity athletics participation, all new student-athletes are required to submit several pieces of information. Electronic forms are included in the table below as well as one copy (front and back) of your insurance card showing proof of primary insurance. Please print and complete the medical history and sports physical forms. Additional Athletics Participation Requirements can be filled out electronically.

***Sports Physical:
- The Sports Physical and Medical History form must be completed by a Medical Doctor. We will not accept sports physicals completed by Nurse Practitioners, Physician’s Assistants, or any other medical professional.
- If you have any current or recent cardiac conditions, and you have seen a cardiologist, please bring a medical clearance note that allows you to participate in sports activity.
- If you have had any recent surgeries that have kept you from participating in sports activity, please bring a medical clearance note from your surgeon that allows you to participate in varsity athletics.
- If we do not have your completed sports physical form on the first day of practice, you will not be permitted to practice until it is completed and you are cleared.
